Set up Feedback Forms and Certificates

Create your own feedback forms and certificates

We LOVE saving event organisers time and effort. Our inbuilt feedback forms and certificates are one of our features which does this, ten fold. Check it out πŸ‘‡

1️⃣ Get Started

β†’ Login to MedAll for Organisations

β†’ Click on 'Events' > Your Event

Note: If you haven't already set up your event, check out this helpful guide first.

2️⃣ Customise your feedback form

On your event admin page, navigate to the 'Next Steps' panel and click 'Create a feedback form & attendance certificate':

If you've previously created templates for your feedback forms, you'll see a drop down which allows you to select one of those. If not, you can simply start to create your form. All feedback forms are customisable meaning you can:

  • Move the question order by dragging them on screen

  • Edit the question by clicking on the three dots on the right side of the question and change the text, question type, star ratings/number of stars/labels etc

  • Delete a question by clicking on the three dots on the right side of the question and select delete

  • Add a new question by clicking 'add a question'

When creating new questions, some helpful guides are:

β†’ Question text is the question you would like to ask on your form

β†’ Question type allows you to change the type of question you ask on your form

β†’ Required allows you to choose if you would like your question to be mandatory (yes) for delegates to complete, or optional (no)

Plus, there is a full range of question types that you can ask:

Text (one-line)

Delegates can provide an answer with a short line of free text.

Text (multi-line)

Delegates can provide an answer with multiple lines of free text.

Rating

Delegates can provide an answer with multiple lines of free text.

Dropdown

Delegates can choose an option from a dropdown list. Enter each dropdown option as a new line when setting up the feedback form.

Dropdown with text (multi-line)

Delegates can choose an option from a dropdown list and provide free text feedback on that option. This works ideally for questions on breakout rooms and workshops, e.g. "Which morning workshop did you go to? What went well in it?" Enter each option as a new line when setting up the feedback form in a similar way to the dropdown-type question above.

Delegates will be asked: "what is your role?" and the choices (undergraduate, doctor, nurse, physiotherapist, occupational therapist) they can select from are shown in the options box.

Delegates can choose an option from a dropdown list and provide free text feedback on that option. 3️⃣ Customise your certificate

β†’ Add your logo

Upload an image of your organisation's logo. We accept standard image files such as .jpg and .png

Tip: We won't crop or stretch your logo so regardless of format it will be undistorted on certificates, but aesthetically a logo which is rectangular, e.g. 1200px width x 630px height fills the space beautifully.

Tip: If you are collaborating with multiple organisations, you can use a tool like Canva to combine your logos into a single image before uploading.

β†’ Add any additional text you need to your certificates

If you need to add text about CPD points or sponsors....or anything else you need to add to your certificates this is where to do it. This will be placed below the date on your certificate but above the signatures, for example the area in the red line is custom text on a certificate

β†’ Add Signature(s)

Upload an image of your signature. We accept standard image files such as .jpg and .png.

Add the name and job title associated with each signature.

Tip: To optimise the aesthetics of your certificates we recommend adding two signatures.

πŸ˜„ When you're happy with your additions, click 'next' to view an example of the feedback form and certificate as shown below, and then 'finish' when you're done.


4️⃣ Share your feedback form

β†’ Change the toggle from "not accepting responses" to "accepting responses":

Tip: When you toggle on accepting responses you will see options to keep a record of the certificates your organisation awards. When this is toggled on, it will also add a secure digital code to your certificates which makes them gold-standard in traceability and anti-forgery. Your organisation will be able to query this code on your account. This option is enabled as default.

By toggling this on, your feedback forms and certificates will be automatically sent to those who attended your event, and after completing the feedback form they will be awarded a certificate automatically.

Your wizard should now look like this:

Note: The feedback form is a one-time link so attendees can't share it with others and you won't need to toggle off responses.

β†’ If you need to send feedback reminders, you can click on the icon below found on the feedback form 'share' page:

To share your feedback form, you can also:

  • Share feedback link with registered attendees: If using MedAll registration you can share the feedback form with those who have registered with a click of a button.

  • Copy the link and paste it into the MedAll Live chat box or circulate the link via email.

  • Share a feedback form using the QR code: This is convenient to put on slides at your event!

  • Send a one-time link via email to attendees: copy delegates email (it can be a list of emails) and then click send feedback requests. It can be a list of emails.

Delegates will be automatically offered the option to generate the attendance certificate that you have created. You can see what it looks like for delegates to complete a feedback form and claim a certificate in this help article.


5️⃣ Review your feedback and find certificates

You can review your feedback by clicking 'feedback' on your event dashboard:

You can see a summary of the feedback on screen, and you can also download a PDF summary or a spreadsheet of line-by-line responses:

You can also check who has been awarded a certificate (and search by name, event or document authentication code) on the "certificate" tab on your organisation dashboard.
