On your MedAll for Organisations account go to "templates" and add a template feedback form and a template certificate - it means if you plan to ask the same questions on your feedback forms and/or add the same logos/signatures to certificates you don't need to set up a new form and a certificate every time you add on-demand content.

Customise your feedback form and then click "Create" your template is now ready to use.

This process is the same for adding a certificate template.

Click on-demand content.

Click the video you want to add a feedback form and certificate to > click create feedback form on the far right of the page under "Next Steps".

Then just select your templates. You can also set up a form here but its much easier and quicker if you plan to add the same form and certificate to lots of videos just to use a template each time. Please follow the steps above to do this.

Follow through the set-up wizard . Before finalising you can view a sample of your feedback form and certificate.

Toggle on your feedback form at the end so that it is "accepting responses", and then people will be able to watch your video, give you feedback on-demand and collect a certificate on-demand when they watch the majority of your video (more than 80%).

Go to events > click on the event > click upload videos > add the video details and upload the video file > when uploaded, click create feedback form. Then just select your templates. You can also set up a form here but its much easier and quicker if you plan to add the same form and certificate to lots of videos just to use a template each time.

Just follow the steps of the wizard, and toggle on your feedback form at the end so that it is "accepting responses", and then people will be able to watch your video, give you feedback on-demand and collect a certificate on-demand when they watch the majority of your video- same as above!
