On feedback forms, is there a way to add a statement to introduce the form including why we are asking for email and full name for example, and that we will ensure data is kept GDPR compliant?

Yes! This is often added by organisations as a consent question as the first question on the feedback form, to ensure GDPR compliance. The statement will then appear at the top of the form and attendees will be actively giving consent.

To do this:

  • add a new question

  • select "dropdown" as the question type

  • select "dropdown" as the question type

  • in the text of the question add the statement of how you are going to use the information, and then gain consent, by adding text at the end of your statement such as: "I consent to this" as a dropdown type question

  • Add "Yes" as the only option response, and make the completion of this question mandatory, if by completing the form you require that consent.

The statement will then appear at the top of the form and attendees will be actively giving consent.
